Ambulance Journey Record


Ambulance Journey Record HK$1.20 per photocopy

Applicants for Ambulance Journey Record (“Record”) should include the following information in their request letters or application forms:

(a) Date, time and location at which the patient was picked up by the ambulance (please provide the full address)
(b) Copy of the identity proof of the patient using the ambulance service (“the data subject”).

The name, correspondence address and daytime contact telephone number of the applicant, should be stated clearly in the application. The application letter/completed Data Access Request Form containing the information stated as above should be sent to the departmental Personal Data Privacy Officer for processing the request.

The requested Record will be mailed to the applicant’s correspondence address after the confirmed receipt of all the required information and payment of the related charges.

Notes: Applicants may be asked to provide additional information to help us process their requests. The department may not be able to process their applications if they do not provide sufficient information or pay the related charges.
